Forms can be found in the canvas portal as well.

Attention Students - Our resource centers are seeing an increase in scheduled students who have been referred by their teacher. If you do not need help from a staff member or have a day pass from your teacher, PLEASE remain in your scheduled study hall. The supervising teacher can and will ask you to leave if you do not need to be in the resource center. Your cooperation is greatly appreciated.
